Referral letters, medical reports, test results or other clinical documents you provide to us.
Our clinical system, appointment system, email system or website forms.

We only use your personal information where we have a lawful basis to do so.
The lawful bases we may rely on include:
Healthcare delivery - We process clinical and health information where it is necessary to provide medical care, diagnosis, treatment and clinical management. This includes processing special category health data under UK GDPR Article 9(2)(h), where processing is necessary for the provision of health or social care.
Contract - We process information where it is necessary to provide the healthcare service you have requested or agreed to receive from us.
Legal obligation - We process information where required to comply with legal, safeguarding, professional, regulatory or statutory duties. This may include duties connected with healthcare regulation, CQC requirements, safeguarding, complaints, medical records and legal requests.
Consent - We may ask for your consent for specific optional uses of your information, such as sending non-essential communications, newsletters or sharing consultation summaries with your NHS GP where consent is appropriate.
Withdraw - You can withdraw consent at any time. This will not affect any lawful processing already carried out before consent was withdrawn.

Our clinical records are stored securely in Cliniko, an encrypted cloud-based clinical records system. Access to clinical records is restricted to authorised users only.

 

8. Who we share your information with


We only share your personal information where there is a clear reason to do so and where the law allows it.

  • Your NHS GP

  • We may share a consultation summary, referral information or other relevant clinical information with your NHS GP where you have given consent, or where there is another lawful basis to do so.

  • Other healthcare professionals

  • We may share relevant information with other doctors, specialists, laboratories, pharmacies or healthcare professionals where this is necessary for your care.

  • CQC and regulators

  • We may share information with the Care Quality Commission or other regulators where required by law or regulatory obligation.

  • Safeguarding authorities

  • We may share information with safeguarding authorities where there is a concern about risk of harm to a child or adult.

  • Police, courts or legal authorities

  • We may share information where legally required, where ordered by a court, or where there is a serious risk of harm.

  • Service providers and processors

  • We may use trusted service providers to support our services, including clinical records, appointment management, secure communication, website hosting, email and administration.

  • Where a service provider processes personal information on our behalf, they must follow our instructions and protect your information appropriately.

  • We will never sell your personal information.

We will never share your personal information with third parties for marketing purposes.

 

9. Relationship with Saxonbrook Medical Practice


Nordicsprings Health Limited operates from The Hope Centre, Saxonbrook Medical Practice, Maidenbower Square, Crawley RH10 7QH.
Nordicsprings Health Limited is legally and clinically independent from Saxonbrook Medical Practice.

 

We do not share your clinical records with Saxonbrook Medical Practice unless:

  • You have given explicit consent.

  • There is a legal obligation to do so.

  • There is a safeguarding concern.

  • There is a serious risk of harm.

 

There is another lawful basis that allows or requires us to share the information.

Where Saxonbrook Medical Practice provides limited administrative support under a formal data processing agreement, they act only as a processor and are bound by confidentiality and data protection obligations.

12. How long we keep your information


We only keep personal information for as long as necessary.
Our retention periods include:

We take data protection and patient confidentiality seriously.
We use appropriate technical and organisational measures to protect your information, including:

  • Secure clinical records held in Cliniko.

  • Encrypted cloud-based storage.

  • Role-based access controls.

  • Two-factor authentication on clinical systems.

  • Password-protected devices.

  • Device auto-lock settings.

  • Restricted access to patient information.

  • Staff training in data protection and information governance.

  • Data breach reporting procedures.

  • No patient-identifiable information stored on personal devices.

  • Audit logging where available.

 

14. Data breaches


If we become aware of a data breach that may pose a risk to your rights and freedoms, we will investigate it promptly.
Where required, we will notify the Information Commissioner’s Office within 72 hours.
Where a breach is likely to result in a high risk to you, we will also inform you as soon as possible.
All data breaches are logged, reviewed and investigated.
